How It Works
Complete the request form with the required information. Our medical records team will verify your identity and authorization before releasing any records.
Please allow 5-7 business days for processing. Complex requests or requests involving a large volume of records may require additional time.
For legal or attorney requests, please include your firm name and any case reference numbers in the message field.
Please note: A fee may apply for records retrieval and processing, in accordance with applicable state regulations. Our team will notify you of any costs before records are released.
